YOU ARE HEREBY INFORMED that, in accordance with Section [Section Number] of the Bylaws of [Name of Corporation], hereinafter referred to as Corporation, the Board of Directors has voted to remove you from your position as [Title of Person being notified] of said Corporation.
This action of the Board of Directors, taken by means of a resolution voted upon at a meeting held on [Date of meeting], effectively removed you from office as of the time of the vote.
A sealed copy of the resolution is attached to this notice; the original is on file in the principal office of the Corporation.
